New Student Orientation Sessions

All students accepted to the Graduate Program are required to complete the on-line orientation.

Doctor of Nursing Practice (DNP) Students

All students accepted to the DNP program for the Fall 2009 semester are required to attend the following on-site orientations:

College of Nursing Student Orientation (DNP students only)

August 17,2009 (1:30pm - 5:30pm) 204 Duxbury Hall - Orientation Session 1
August 17, 2009 (5:30pm - 7:00pm) 470 Duxbury Hall - Reception with faculty
August 18, 2009 (8:00am - 11:30am) 470 Duxbury Hall - Orientation Session II

FSU New Graduate School Orientation and Blackboard Workshop (DNP Students Only)

August 18, 2009
Students will need to self-register for the orientation and are strongly encouraged to register for the "Using Blackboard" workshop. For orientation registration information, go to http://gradschool.fsu.edu/orientation/

When requesting your appointment for fingerprinting through CertifiedBackground.com, you will need to enter the following information:

NOTE: Students will receive the Chain of Custody Form that is required to complete the drug screening once payment has been remitted online.

Essential Functions Required of Nursing Student Form
Current TB/PPD Test Results (Two-step Mantoux required) Test should be done between August 7 and August 14.

Students with positive results will need to provide evidence of a clear cheat x-ray and complete the Positive PPD Questionnaire

Authorization for the Use and Disclosure of Protected Health Information and Personal Background Information Waiver Form
